Campaign Form Templates

Forms collect the information you need from visitors. Choose from presets or create your own.

Preset Templates

Industry-specific templates ready to use:

• Contact information forms
• Inquiry forms
• Lead capture forms
• Event registration forms

Preset templates include common fields:

• Email (required)
• Name fields
• Phone number
• Message/Comments

Custom Templates

Create forms tailored to your needs:

1. Go to Farm > Form Templates
2. Click Create New
3. Add and configure fields
4. Save your template

Selecting a Template

In the campaign builder:

1. Go to the Form tab
2. Browse available templates
3. Click to select
4. Preview the form fields

Field Types Available

• Email Input

• Text Area
• Dropdown Select
• Checkbox
• Radio Buttons

Contact Mapping

Form fields map to contact properties:

• Email → Contact email
• First/Last Name → Contact name
• Phone → Contact phone
• Custom fields preserved in submission
